How does an external party request a medical record?

If an external party, for example a legal representative or insurance company, needs to order a medical record request, you do this by filling in the form on this page.

Medical record copies are classified information. This means that when you order a medical record copy for another person, you must submit a power of attorney from the person you are ordering on behalf of, which gives consent between the patient and the person requesting the information. The consent means that the patient approves that the information may be ordered and potentially disclosed to another person.

When requesting medical record copies, a harm assessment is always carried out on the requested medical record information. A harm assessment means that an assessment is made of the information, so that what is written cannot be perceived as harmful or cause suffering for the patient or relatives of the patient.
